Sports physicals are usually required once a year.

Sports physicals for junior athletes, school and camp are usually required once a year or before participating in any physical activity for school, sports teams and  summer camps.

What is a Sports or School Physical Exam?

The Specific requirements may vary according to the organization requesting the physical. Our sports or school physicals are conducted by our board-certified nurse practitioner or physician's assistant.

  • During the physical we review your child's medical history
  • We conduct a hands-on physical examination
  • We also provide information regarding concussions and how to tell if your child has one
  • We provide the child's primary care provider with a summary of the visit
  • We will also complete any forms required for sports or school participation

Please Note: If the school, team or other organization requires specific forms to be filled said forms must be provided by the parent or guardian.

Why Does My Child Need a Physical?

Regular physical exams provide an opportunity for your healthcare providers to discuss any emergent health issues with children, teens, and their parents or guardians. Physical exams help us make sure that your child is in good health and physically able to participate in school or sporting activities. Many states require physical exams once a year or before participating in physical activities at school, camp or other community organization.

What You Need to Bring

At the time of the physical you will need to bring with you any required forms for us to complete. Also bring your child's contacts or glasses, if any.

Minors must be accompanied by an adult and have parental or guardian permission.

Paying for your child's physical exam

Most sports or school physicals are not covered by insurance, please check with your carrier to make sure. Payment by cash or credit card is required at the time of services rendered. We do not accept personal checks.
